Dear HHS Employee,
Welcome to the PSC Child Care Subsidy Program (CCSP) resource center. For your convenience, the following links have been provided to help you manage and submit required documentation related to your participation in the program:
- PSC Child Care Subsidy Apply Now – Use this link to securely upload required CCSP forms (e.g., applications, provider documentation, or eligibility updates).
- PSC Child Care Subsidy Monthly Invoice & Payment Upload – Submit your monthly invoices and supporting documentation for child care payments through this portal.
- PSC Child Care Subsidy Eligibility Information – Detailed eligibility requirements and income qualifications for HHS offices participating in the PSC Child Care Subsidy Program.
If you meet the CCSP eligibility criteria for your respective office, you will be required to submit the following documentation as part of your application:
Employee Forms
- HHS-698 – Application for Child Care Subsidy Program
- Standard Form 50 (SF-50) – Verification of HHS employment
- Recent Federal Income Tax Returns – For each parent/guardian/spouse
- Two Most Recent Pay Statements – For all eligible parents/guardians/spouses
- IRS W-2 Forms – For each parent/guardian/spouse
- Birth Certificates – For each child enrolled in the program
Provider Forms
- OPM-1644 – Child Care Provider Information for Subsidy Program
- IRS W-9 Form – Taxpayer Identification Number form for providers
- Child Care Provider License – Current and valid documentation from the provider
- Rate Sheet – From the child care provider outlining costs
